چکیده

Increased evidence suggests that opticospinal multiple sclerosis (OS-MS) preferentially found in Asians is a distinct disease entity which differs from the conventional or universal form of MSin manyaspects. The following characteristics have been reported in OS-MS;1) severe and selective involvement of the optic nerve and spinal cord, 2) paucity of brain MRI lesions, 3) frequent occurrence of spinal cord atrophy on MRI, 4) marked pleocytosis in the cerebrospinal fluid (more than 50 cells/|iil), 5) a strong association with HLA-DPB 1 *0501 allele, but not with HLA-DRB1*1501allele which is a susceptibility gene for the conventional form of MS(1, 2). Yamasaki et al in this issue of Internal Medicine added a new feature to the abovementioned list, i.e. , a frequent occurrence of hyperprolactinemia (3).

Hypothalamus syndrome in opticospinal multiple sclerosis.

This study describes a case of a patient with OSMS who presented with somnolence, periodic fever, memory impairment, and amenorrhea. Serum prolactin levels were found to be higher than normal. MR imaging showed a bilateral involvement of the hypothalamus. The clinical presentation, laboratory examination, and MR imaging findings suggested a diagnosis of hypothalamus syndrome in this patient.

Intrathecal activation of the IL-17/IL-8 axis in opticospinal multiple sclerosis.

There are two distinct subtypes of multiple sclerosis in Asians, opticospinal (OS-multiple sclerosis) and conventional (C-multiple sclerosis). In OS-multiple sclerosis, selective and severe involvement of the optic nerves and spinal cord is characteristic, though its mechanisms are unknown. The present study aimed to find out possible differences in the cytokine/chemokine profiles in CSF betwee...

Modification of Multiple Sclerosis Phenotypes by African Ancestry at HLA.

BACKGROUND In those with multiple sclerosis (MS), African American individuals have a more severe disease course, an older age at onset, and more often have clinical manifestations restricted to the optic nerves and spinal cord (opticospinal MS) than white persons. OBJECTIVE To determine whether genetic variation influences clinical MS patterns. DESIGN Retrospective multicenter cohort study...

Helicobacter pylori infection is a potential protective factor against conventional multiple sclerosis in the Japanese population.

Persistent Helicobacter pylori (H. pylori) infection is a chronic inflammatory stimulus to hosts with an inverse correlation to atopic disorders. In this study, a total of 105 consecutive multiple sclerosis (MS) patients were divided into 52 opticospinal MS (OSMS) and 53 conventional MS (CMS), and their sera, along with those from 85 healthy controls (HC), were examined by an enzyme-linked immu...

Preliminary Studies on the Clinical Features of Multiple Sclerosis in Korea

BACKGROUND AND PURPOSE Multiple sclerosis (MS) in Asians is characterized by frequent involvement of the spinal cord and optic nerve and low prevalence rates, but even the most fundamental epidemiologic findings and unique clinical features of MS patients in Korea have not been studied extensively. We performed this study to establish the clinical spectrum of MS patients in Korea. METHODS Six...
